Liability Release Form

Release of Liability, and Photo Release Form.
This release form must be completed by all people participating in Blue Mountain Outfitters river trips. Each adult must fill out a seperate form. Children under 18 years of age should be listed on the bottom of the form of the adult responsible for them (parent or legal guardian) with the adult signing both locations. Please read the document prior to signing, and initial each box on the right hand side, indicating that you have read and understood each section of the form.
